Should you choose GKV or PKV in Germany?
Most expats should stay in public insurance (GKV): it costs about 8.75% of gross as your employee share in 2026, insures non-working spouses and children free, and never re-prices you for age or health. Private (PKV) wins mainly for young, healthy, high-earning singles staying under about 10 years. PKV is near-irreversible after 55.

Our verdict for your profile
PKV
Young, high-earning, single and not staying forever: this is the one profile where PKV genuinely wins. Lock a good tariff, and leave Germany before the age curve catches up.
Monthly cost over the next 10 years (indicative)
| Year | GKV (your share) | PKV (household, indicative) | Cheaper |
|---|---|---|---|
| Now | €582 | €450 | PKV |
| +2 yrs | €605 | €482 | PKV |
| +4 yrs | €630 | €516 | PKV |
| +6 yrs | €655 | €553 | PKV |
| +8 yrs | €681 | €735 | GKV |
| +10 yrs | €709 | €787 | GKV |
GKV = income × 8.75% (employee half of 14.6% + 2.9% average Zusatzbeitrag) + care insurance, capped at the contribution ceiling. PKV figures are indicative 2026 quote bands for mid-tier tariffs with a 3.5%/year premium drift; real quotes depend on health history. Not financial advice.
Frequently asked questions
Can I switch back from private to public health insurance?
It is deliberately hard: under 55 you must get your income under the threshold (JAEG) or become employed below it; over 55 returning is nearly impossible. Treat choosing PKV as close to irreversible, especially if you might stay in Germany long-term.
Why is PKV cheap now but risky later?
PKV premiums are risk-based and rise with age and healthcare inflation, while GKV stays a percentage of income and covers non-working spouses and children free. A €350 premium at 30 routinely becomes €700-900 in retirement. Short-stay high earners can still win with PKV.
What income do you need for private health insurance in Germany?
Employees need gross income above the Jahresarbeitsentgeltgrenze (JAEG) to choose PKV. The 2026 threshold must be verified before you decide - it moves every January. Freelancers and the self-employed may choose PKV at any income.
